Pediatric Toxidrome Simulation Curriculum: Salicylate Toxidrome

This scenario teaches learners to recognize and manage pediatric salicylate toxicity and its complications, including metabolic acidosis, respiratory alkalosis, electrolyte abnormalities, and cerebral dysfunction. The scenario is simulation-based with an integrated team-communication focus. Learners will identify the signs and symptoms of a salicylate toxidrome and learn the appropriate evaluation for, and management of, salicylate toxicity. The learners should recognize the seriousness of the situation and organize a full resuscitation team. The primary complications for the team to recognize and manage are acid/base and metabolic disturbances, as well as altered mental status. We included extensive preparatory material to help the instructor prepare the environment and learners. The debriefing tools have been tailored specifically for this scenario with advice on how the instructor can edit them for different learners. There are options that allow the difficulty of the case and the goals for evaluation and management to be changed according to the needs of the learner group. This resource tries to anticipate most questions and issues that could arise based on the issues we’ve encountered, while also allowing each instructor the flexibility to tailor their educational objectives to their learners. Particular examples are the instructor evaluation/debriefing forms. It is difficult to address more than three of these issues per session. We chose to include many more than three potential issues, allowing each instructor to focus on those most relevant to their group of learners. This resource can be used in isolation or as a component of our pediatric toxidrome simulation curriculum to provide more comprehensive pediatric toxicology and resuscitation training.

By the end of this lesson, the learner should be able to:

  1. Demonstrate recognition and management of altered mental status in a pediatric patient.
  2. Demonstrate recognition and management of salicylate toxicity in a pediatric patient.
  3. Demonstrate efficient, effective teamwork and communication skills.
